When the shuffle enemies item is used, it will shuffle all enemies following the player in a randomized order. The shuffle enemies item is useful for delaying a current fight between a difficult enemy and allows the player to delay them, however, this tactic cannot be considered completely effective as it shuffles all enemies randomly and can still give the player a difficult enemy.
